OSAA Athletic Director Checklist - Spring 2009

The OSAA staff has compiled the following list of items as reminders for all Athletic Directors. As the Association continues to communicate electronically, it is imperative that the OSAA office have an accurate email address for every Superintendent, Principal, Athletic Director, Activity Director, AD Secretary, Varsity Head Coach, and any other appropriate school personnel. School Sports Pre-Participation Exam Form
DEADLINE: Prior to participation
REQUIRED every two years for all students 7th-12th grade prior to participating in extracurricular sports; Oregon State Board of Education mandates use of this form.
Eligible Student Transfer Certificate
DEADLINE: Prior to participation
REQUIRED for each eligible transfer student new to your school; maintain on file at school.
School Representation Eligibility Certificate
DEADLINE: Prior to participation
REQUIRED for a student who does not attend the school he/she wishes to represent; maintain on file at school.
Spring Plans & Brackets: Baseball | Golf | Softball | Tennis | Track & Field
DEADLINE: Prior to first practice
General sport information, NFHS rules, OSAA policies and important dates; direct coaches and parents to download for each sport.
Student, Coach, AD, and Cheer Coach Eligibility Report
DEADLINE: Prior to first contest
Verifies all participating students are eligible, all AD's and coaches are certified through ASEP or the new NFHS Coaches Education program, and all cheer coaches attended an annual Cheer Safety Clinic; Principal's signature REQUIRED.
Adding a New Sport After the Deadline
DEADLINE: Prior to first contest
Review the list of districts for each sport on the website to make sure your school appears; this form is REQUIRED for any school wishing to add a sport that they didn't already notify OSAA as wanting to offer for the current year.
School Information Form
DEADLINE: Friday, April 10, 2009
REQUIRED to update contact and personnel information for your school; note request for Varsity Head Coaches email addresses. This form MUST STILL be completed even if no personnel changes occurred at the school in the past school year.
Financial Need Application
DEADLINE: Thursday, March 12, 2009
For students in need who are currently participating in speech, music, golf, tennis, track, baseball, or softball.
Spring Pre-Selection of Athletics/Activities
DEADLINE: Friday, April 10, 2009
Shows which Spring activities your school will offer in Spring 2010; will be voted on by Executive Board on May 4, 2009.
Dairy Farmers of Oregon Academic All-State Program
DEADLINE: Friday, April 10, 2009
Awards program recognizing cumulative team GPA in speech, music, golf, tennis, track, baseball, softball.
OSAA Scholar Program
DEADLINE: Friday, May 1, 2009
Recognizes graduating seniors (Scholar Athletes and Activity Scholars) with cumulative 3.50 GPA or higher with certificates.
NFHS Spring Participation Survey
DEADLINE: Friday, April 10, 2009
REQUIRED survey detailing # of students (boys, girls, home school, and foreign) participating in your Fall sports programs; seasonal state championship reimbursement will be sent only to those schools that have completed this important NFHS survey.
Spectator Conduct Pre-Season Meeting
SUGGESTED to inform coaches, students, and parents about new OSAA policies regarding sportsmanship at high school events.
Rules Book Order Form for 2009-10
DEADLINE: August 1, 2009 (for Fall sports)
Mailed seasonally (Fall – early August, Winter – late October, Spring – early February); orders are processed based on availability.
If one of your teams qualifies for the state championships, remember these items:
Discount Ticket Form (with payment)
DEADLINE: Early May
For track, baseball, and softball; discount tickets good for 1st round through final sites; for schools only, not general public.

DEADLINE: Three weeks prior to cutoff date
Includes event sites and times, plus information about tickets, parking, maps, souvenirs; direct coaches and parents to download each sport's information.

Instructions, ticket signs & reports, PA script for ADs of host schools; ADs must download from sport's page on OSAA website.
