OSAA Incident Report

Create a New Discriminatory or Harassing Behavior Incident Report for 2024-25


This form is intended for OSAA-certified officials to report to the OSAA incidents involving discriminatory or harassing behaviors not associated with an ejection report. These online incident reports can only be filled out by an official or a commissioner. This form is not open to the public to submit complaints.

This form should be filled out anytime a contest is stopped or interrupted by discriminatory or harassing behavior and the Immediate Incident Response Protocols are implemented by officials.

Incident Report Form Process

  1. The Official or Commissioner fills out and submits an online Incident Report.
  2. The OSAA is notified of the incident.
  3. The Schools are contacted by the OSAA for follow-up if needed.
