OSAA Ejection Report

Create a New Ejection Report for 2023-24


The OSAA needs to be informed of every high school player or coach ejection. Online ejection reports are to be initially filled out by the official or commissioner.

Ejection Report Form Process

  1. Official fills out a preliminary online Ejection Report.
  2. Commissioner is notified and reviews the form before submitting.
  3. School is notified of ejection and reviews information.
  4. OSAA is notified if school is appealing the suspension/fine.
